Referee Assessment

To be completed by sanctioned PRA assessors only
 This form will be used to provide a game assessment of a referee's performance as viewed by the assessor assigned by the PRA assignor. Those observed will receive a copy of the report that will also be filed with the PRA.assignor as well as the the assessor who submits the report.
