Print two copies, submit one to the coach of the home team on
the day of the contest and keep the other, initialed by the coach.
(Attach W-9 if applicable, see recommendation below)

 Required to be signed and submitted to the PRA as a condition of membership.

PRA recommends attaching a completed, signed copy each time you referee at a school for the first time.
Bring a W-9 to tournaments IF you have not already submitted one to the host school.

 Submit to the host school ONLY in the event that payment has not been received within a reasonable time period (i.e. 45 days after conclusion of tournament) . Keeping a file copy is recommended.
